Library Parent Consent Form

According to the Data ProtectionPolicies of the Community & School Library and the Hellenic Schoolof St Peter & St Paul, we need to have your consent on using your child’s personal data for library purposes. Please return this form completed for each of your children within the school registration package. This will be then handed to the library staff and saved in the library records.

If this form is not completed and returned, it will be taken as NO consent.

I give consent for:

• my child’s full name, date of birth and contact details to be used on the library’s electronic lending system in order to become registered member.
• the library to handle my child’s borrowing, reservation and overdue history.
• the library to exchange information on my child’s reading activity, preferences and progress with the teaching staff and myself. I understand that this aims to improve my child’s reading skills.
• the library to contact me for my child’s library related issues.

I DO NOT GIVE CONSENT for my child’s details to be used by the library. I understand that my child will not be a member of the library and will not participate in the Library-School Programme.
